When to Apply
To help make Healthy San Francisco work for everyone, the program is being phased in starting with those who have the lowest income. Following is the schedule for when you may be able to apply:
You may apply beginning September 17, 2007 if you are ALL of the following:
- A current patient with an existing appointment at any of the 27 participating clinics.
List of participating clinics; - Living on a combined family income at or below 100% of the Federal Poverty Level.
See if you meet the income requirement for this enrollment phase; - A San Francisco resident who can provide proof of San Francisco residency;
- Uninsured for at least the past 90 days;
- Not eligible for public insurance programs such as Medi-Cal, Healthy Families,
or Healthy Kids & Young Adults™; - Between the ages of 18 and 64
You may apply beginning January 2, 2008 if you are ALL of the following:
- Living on a combined family income at or below 300% of the Federal Poverty Level.
See if you meet the income requirement for this enrollment phase; - A San Francisco resident who can provide proof of San Francisco residency;
- Uninsured for at least 90 days;
- Not eligible for public insurance programs such as Medi-Cal, Healthy Families,
or Healthy Kids & Young Adults™; - Between the ages of 18 and 64
Eligible residents may join Healthy San Francisco regardless of immigration status or pre-existing medical conditions. There is NO LIMIT to the number of qualified residents who can enroll. If you qualify, there is room for you in the program. We appreciate your patience as our dedicated clinics work to enroll qualified applicants as quickly as possible.
