Healthy San Francisco is a program created by the city of San Francisco that makes health care services accessible and affordable for uninsured residents.  Healthy San Francisco offers a new way for San Francisco residents who do not have health insurance, to have basic and ongoing medical care.

The Medical Home Network

When you enroll in Healthy San Francisco you will choose a Medical Home.  A Medical Home is the facility (in most cases, a clinic) that will assign you your own physician who will provide your medical care.

The current Healthy San Francisco Medical Home is comprised of San Francisco Department of Public Health clinics, San Francisco Community Clinic Consortium clinics, and private providers.

If you are interested in becoming a Healthy San Francisco provider independently, please contact one of the above medical homes as we do not currently contract with providers directly.
