The Medical Home Network

When you enroll in Healthy San Francisco you will choose a Medical Home.  A Medical Home is the facility (in most cases, a clinic) that will assign you your own physician who will provide your medical care.

The current Healthy San Francisco Medical Home Network is comprised of San Francisco Department of Public Health clinics, San Francisco Community Clinic Consortium clinics, and private physician groups and other providers.
