How to Apply
STEP 1: Determine if you qualify to apply for the program.
There are certain requirements for participation in Healthy San Francisco. Find out if you may qualify.
If you think you might qualify, you will need to make an appointment at one of our Enrollment Sites.
STEP 2: Make an enrollment appointment with a Certified Application Assistor.
When you make an appointment at an Enrollment Site, you will meet with a Certified Application Assistor that will determine if you are eligible for Healthy San Francisco based on the information you provide.
To find an enrollment site, select a clinic from the pull-down menu below and call the phone number listed to make an appointment to complete a Healthy San Francisco application.

STEP 3: Complete a Healthy San Francisco Application at your enrollment appointment.
In order to verify your eligibility and complete your application, be sure to bring all required documents to your appointment. At your appointment, the Certified Application Assistor you meet with will inform you whether or not you qualify for the program. For a list of required documents, click here.
Only a Certified Appliciation Assistor can complete and submit a Healthy San Francisco application.
If you qualify, you will receive a Summary Sheet from the Application Assistor with more information about your Medical Home and instructions on how to access medical services.
Within the next couple of weeks, you will receive a Participant ID Card and a Participant Handbook in the mail.
If you are required to pay a Participant Fee, you will receive a bill in the mail within the next month.
