Healthy San Francisco is a program created by the city of San Francisco that makes health care services accessible and affordable for uninsured residents.  Healthy San Francisco offers a new way for San Francisco residents who do not have health insurance, to have basic and ongoing medical care.

How to Apply

STEP 1:  Determine if you qualify to apply for the program

With the large number of people living in San Francisco without insurance, the Healthy San Francisco program is being phased in over time.   Find out if you may qualify to apply.


STEP 2:  Make an appointment to apply

  • Select a clinic from the pull down menu below, and call the phone number listed to make an appointment to complete a Healthy San Francisco application.  Be sure to bring all required documents to your appointment.

STEP 3:  Complete a Healthy San Francisco Application.

Only a Certified Application Assistor can complete and submit a Healthy San Francisco application. After you meet with the Application Assistor, he or she will inform you whether or not you qualify for the program. If you qualify, you will receive a Summary Sheet from the Application Assistor with more information about your Medical Home and instructions on how to access medical services.

Within the next couple of weeks, you will then receive a Participant ID Card and a Participant Handbook in the mail.

If you are required to pay a Participant Fee, you will receive a bill in the mail within the next month.
