Your Medical Home

When you enrolled in Healthy San Francisco you chose a Medical Home.  A Medical Home is the facility (in most cases, a clinic) that assigned you your own physician who will provide your medical care.

The current Healthy San Francisco Medical Home is comprised of San Francisco Department of Public Healthand San Francisco Community Clinic Consortium clinics.
