How to Make an Appointment
It’s simple – just call your Medical Home. The phone number and address are printed on your Healthy San Francisco Participant ID Card. If you need help finding your Medical Home try locating it on this Interactive Google Map, or call Healthy San Francisco Customer Service at (415) 615-4555, Monday through Friday, 8:30am to 5:30pm.
Be sure to bring your Participant ID Card to your appointment. If you can’t keep the appointment, call your Medical Home and cancel or reschedule it with at least 48 hours notice.
