Durable Medical Equipment

If you need it, your Medical Home doctor will write a prescription for medical equipment such as a wheelchair or crutches. These are called durable medical equipment. Check with your Medical Home about where to take a prescription for durable medical equipment. The telephone number for your Medical Home is located on your Healthy San Francisco Participant ID Card.
