Participant Fees
As a Healthy San Francisco participant, there are some fees you may need to pay.
Participant Fee
Depending on your income, you may be required to pay a Participant Fee paid four times a year. The amount you pay depends on your household income. If your income is below a certain amount, you may not be required to pay this fee at all. If you do have to pay a Participant Fee, you’ll receive a bill every three months. You must pay this bill to remain in the program. If you do not pay, you will be disenrolled from Healthy San Francisco.
Point of Service Fees
A Point of Service Fee is what you pay for medical services at the time you receive them. For example, you may need to pay a fee each time you visit the doctor, go to the emergency room, or have a prescription filled. This amount will depend on your Medical Home and your household income, and what medical service you are getting. If your income is below a certain amount, you may not have to pay a Point of Service Fee for most services. For more information on specific fees, please contact your Medical Home. The number is on your Participant ID Card.
If after enrolling in the program, you have either a change in your household income (includes income and liquid assets) or a change in the number of people in your household, please contact Healthy San Francisco Customer Service to see if a change in your fees might be needed. Healthy San Francisco Customer Service will direct you to your medical home or enrollment site to update your application if a fee modification is needed. A decrease in your income/liquid assets or increase in the number of people in your household may result in lower fees. An increase in your income/liquid assets or decrease in the number of people in your household may result in higher fees.
