Get a New Participant ID Card

If you are new to the program, please allow up to 2 weeks to receive your Participant ID Card in the mail.  

To request a new ID Card online, please fill out the following form below. We’ll send you a new one at no charge if:

  • Any information is incorrect
  • Your card is lost or stolen
  • You change your Medical Home
  • Your Medical Home address or other information changes

You should receive your new card within 2 weeks of your request.
