Healthy San Francisco is a program created by the city of San Francisco that makes health care services accessible and affordable for uninsured residents.  Healthy San Francisco offers a new way for San Francisco residents who do not have health insurance, to have basic and ongoing medical care.

About ID cards

Healthy San Francisco Participants are mailed an identification card, called a Participant ID Card, when they enroll, renew, or when they change Medical Homes. Their ID card lets their Medical Home know that they are enrolled in Healthy San Francisco.  Participants are advised to carry this ID Card at all times, but most importantly when:

  • Visiting their Medical Home
  • Accessing medical services of any kind
