Healthy San Francisco
Healthy San Francisco is a program that was created by the City to provide affordable medical care to uninsured people living in San Francisco. Participants choose a Medical Home to go to when they are sick, as well as for preventive care to help keep them healthy.
While Healthy San Francisco provides basic and ongoing medical care, the program is not health insurance. Healthy San Francisco offers a limited network of health care providers. If your employee uses doctors, hospitals or other medical services outside the program’s scope, Healthy San Francisco will not pay those costs. So if you currently offer health insurance, do not drop it. Or if you are considering securing insurance to comply with the Health Care Security Ordinance, look into it.
To sign up for the City Option, you will first need to register as an employer via our Employer Portal. Then, you will:
- Give us a list of the employees for whom you will be making a payment to the City Option
- Itemize the amount of money you will deposit on behalf of each employee; and mail in one payment for all employees or pay online through the Portal.
After your payment is received, each of your employees will receive a letter informing them where to go to apply for the program, bringing with them all required documentation.
If your employee is deemed eligible for Healthy San Francisco, he or she may receive a 75% discount on participation fees when they enroll in the program at a designated enrollment site. Enrollment fees are calculated as follows:

Employees may begin accessing health services immediately, and will receive an ID card and other program materials in the mail.
If your employee is deemed ineligible for Healthy San Francisco, the funds you contribute are sent to a Medical Reimbursement Account.
Your employees will either be assigned to Healthy San Francisco or receive a Medical Reimbursement Account, based on the information you provide. For more information on Healthy San Francisco eligibility rules and other requirements, please visit the Eligibility Section of this website. If your deposits are assigned to Healthy San Francisco and your employee is deemed ineligible for that program at the time they apply, your employee may request that the funds be transferred to a Medical Reimbursement Account.
Please note: Only uninsured San Francisco residents between the ages of 18-64 qualify for Healthy San Francisco.
