加入 Healthy San Francisco 計劃後，您需選擇一間指定醫療診所。指定醫療診所是指派為您提供醫療護理的私人醫生的機構 (大多數情況下為診所)。

目前 Healthy San Francisco 指定醫療診所由三藩市公共衛生局 (San Francisco Department of Public Health ) , 三藩市社區診所協會 (San Francisco Community Clinic Consortium clinics) 的診所, 和私有醫療團體組成。
