若要在線上申領新會員卡，請填寫以下表格。在以下情況下，我們將免費寄給您一張新會員卡：

  • 資料不正確
  • 您的卡已丟失或被盜
  • 您已變更您的指定醫療診所
  • 您的指定醫療診所的地址或其他資料發生變更。

您將在提出申請後 2 週內收到新會員卡。
